Anaesthetic requirements in patient monitoring.

چکیده

According to the New English Dictionary, to monitor means to give warning of impending danger. Fundamentally monitoring techniques in anaesthesia serve three main purposes. First they protect the patient by ensuring that the anaesthetist is made aware of any disturbance in physiological patterns beyond acceptable limits. Secondly, the pattern of the patient's response to the effects of anaesthetic and allied drugs or to the effects of surgical interference can be assessed with a view to reversal, curtailment or withdrawal if necessary. Thirdly, the data derived from monitoring specific groups of patients can be used to provide a better understanding of the effects of a given technique or procedure. In anaesthesia the problems of monitoring are conveniently considered under two headings, those related to the operating room and those concerned with management in the post-operative phase in the recovery ward or the intensive care unit.

Bispectral Index Monitoring in a Severe Comatose Patient During Surgery

A comatose patient required a tracheostomy. His Glasgow Coma Score was 4 and he was intubated with mydriatic and isochoric pupils, without corneal and light reflexes. Two previous EEGs showed signs of severe and diffuse encephalopathy (arreactive delta and theta rhythms) and there were no somatosensory evoked potentials recordable at the scalp. When the tracheostomy procedure was carried out, t...

An integrated system for significant anaesthetic events monitoring

Patient safety assessment in anaesthesia increasingly relies on the monitoring of frequent but merely undesirable events, like hypotension. We report on the design and implementation of such a monitoring system, where 8032 patients were included over a three years period. Thirty two 'Significant Anaesthetic Events' were defined and their occurrence was routinely collected for each patient. A to...

Intravenous phenytoin and percutaneous arterial cannulation: the purple-glove syndrome.

propofol and remifentanil has been reported to be successfully used in a patient with narcolepsy who underwent cardiac surgery [6]. The authors of that report considered that perioperative monitoring of the patient’s hypnotic state would have added valuable information, but this kind of monitor was not available in their department [6]. There are limited data about inhalational anaesthesia in n...

Mortality associated with anaesthesia: a qualitative analysis to identify risk factors.

From a prospectively defined cohort of patients who underwent either general, regional or combined anaesthesia from 1 January 1995 to 1 January 1997 (n = 869 483), all consecutive patients (n = 811) who died within 24 h or remained unintentionally comatose 24 h after anaesthesia were classified to determine a relationship with anaesthesia. These deaths (n = 119; 15%) were further analysed to id...

Preoperative anxiety and pain sensitivity are independent predictors of propofol and sevoflurane requirements in general anaesthesia.

BACKGROUND Psychological factors are thought to drive inter-patient variations in anaesthetic and analgesic requirements. This cross-sectional study investigated whether preoperative psychological factors can predict anaesthetic requirements and postoperative pain. METHODS Before total thyroidectomy, 100 consecutive women completed the Spielberger's State-Trait Anxiety Inventory (STAI) and th...
